People with long-term conditions or chronic health problems can benefit from having an Anticipatory Care Plan in place. This workshop explains the process and options in more detail.

This course is for anyone caring for someone with concerns about how to express their or the person they support’s wishes of medical treatment and care.
Outcomes of this online session
By the end of the session we will have explored:
- A description of what Anticipatory Care Planning is, who it’s for, the benefits and the challenges.
- An explanation of the contents of an Anticipatory Care Plan.
- How to get support with Anticipatory Care Planning.
Rebecca Caulfield, Carer Training Officer, VOCAL. Rebecca has worked at VOCAL for 6 years and now manages the Carer Training programme at VOCAL. She runs a number of face-to-face and online courses for Carers and Wendy Brooks, Carer Support Worker, VOCAL. As part of her role at VOCAL Wendy supports carers to complete Anticipatory Care Plans.
